- Y.E.S. Hours Form (Youth Engaged in Service)
Students who complete 50 hours of community service per each school tier (K-5, 6-8 and 9-12) are eligible to receive the Youth Engaged in Service (Y.E.S.) award at the end of grade five, grade eight and grade 12.
Community Service hours can be earned by volunteering (not being paid) to help others. Routine school chores (clapping erasers, cleaning cafeteria tables) and household chores (shoveling snow or washing dishes at the student’s own house) do not count as community service hours.
Please contact Jean MacLeod in the Assistant Superintendent’s office 781-937-8233 Ext 11208 with any questions. CORI Form
All volunteers must pass a CORI check issued by Woburn Public Schools. Please fill out this CORI Form and bring it, along with a copy of a government issued ID to school between 9:00am and 2:00pm. A staff member will then make a copy of your ID and submit your CORI application. There are also copies of the form you can fill out at the school. Please direct questions to Leanne Venezia, School Secretary, at lvenezia@woburnps.com.
Please note: it can take up to TWO WEEKS for a CORI to be processed.
